Abstract

Comparison of Transversus Abdominis Plane Block versus Intrathecal Clonidine in Lower Segment Caesarean Section - A Prospective Observational Study

Background and Objective: In the majority of abdominal procedures, transverse abdominis plane block, one of the fascial plane blocks, has been used extensively to treat post-operative pain. Ultrasound has gained popularity in the management of cesarean patients due to its ease of use and reduced adverse effects.To assess the effectiveness of ultrasound guided transverse abdominus plane block with intrathecal clonidine for post surgical pain control in lower segment cesarean patients.

Material and Methods: This prospective observational study was done in a secondary training hospital in department of anaesthesiology from November 2023 to October 2024. Our 50 patients were split into two groups of 25 each (TAP and ITC). During spinal anesthesia, the TAP group received 20 ml of 0.25% bupivacaine on each side in addition to 2 ml of 0.5% bupivacaine, while the ITC group received 75 mcg of intrathecal clonidine and 2 ml of 0.5% bupivacaine. Post operative VAS score during rest and movement over 24hrs, duration of analgesia and total dose of tramadol consumption in 24hrs were recorded.

Statistics: A statistically significant P value was defined as 0.05 or 0.001 in the Statistical Package for the Social Sciences software (SPSS version 22.0, IBM Corp., Chicago, IL, USA 2013).

Results: VAS score and total tramadol dose over 24hrs were considerably lower in ITC group compared to TAP group with P value < 0.0001.

Conclusion: When it comes to treating post-operative pain in lower segment cesarean patients with acceptable hemodynamic instability, intrathecal clonidine works better than transverse abdominus plane block.
